A 30-year-old patient was brought by his foreman at around 9 am shift. The patient had been having chest pain since the previous night. It was the central chest pain, more like heaviness. The pain was associated with sweating, nausea, and radiation to the jaw and left arm area. That means the symptoms were typical of a heart attack. I advised him on ECG.

ECG revealed ischemic changes in two leads. As the next step, I advised them to do the cardiac enzyme test. Due to the high price, they were initially hesitant but eventually agreed. So, the blood was sent for Trop T. In the meantime, the foreman told me that he was also a cardiac patient. Before, he was referred from our clinic to the higher center and there he spent a big amount of money without much benefits. Later, he (foreman) went to his country and placed a few stents in his heart. So, the forearm was asking me not to refer him to the higher center if the blood test result came positive for a heart attack!

The cardiac enzyme was elevated. It was an acute Myocardial infarction, aka heart attack (NSTEMI). We loaded the patient with some medications immediately. The next step was to escort him to the higher center via an ambulance. But, the forearm refused. He was trying to send the patient to his home country. Having an active chest pain, it’s against protocol to send a patient to a place other than a hospital. The patient needed immediate management in a cardiac facility. Sending him to his home country was time-consuming.

The forearm called for backup, his sponsor! The sponsor also wanted to send the patient to his home country, instead of taking him to the higher center. We insisted but failed. Finally, the sponsor signed a paper mentioning that they took the patient at their own risk, against protocol. They left. The sponsor told us that he would take him to the higher center himself. But, we didn’t buy the story! They wouldn’t take him to the higher center in Oman. They would send him to his home country with active chest pain! Poor and unfortunate patient!
